California’s Medicaid program will stop paying for GLP-1s for weight loss
Facing a budget gap, California cut Medi-Cal coverage of GLP-1 drugs used for weight loss, while keeping them for diabetes.

The cut
California’s budget for 2025-26 ended Medi-Cal coverage of GLP-1 medicines for weight loss from 1 January 2026. Use for diabetes remained covered.
Why
The state faced a large budget gap, and spending on these drugs had grown quickly. Weight-loss coverage was an optional benefit, which made it easier to cut.
Who is affected
- Adults on Medi-Cal who use these drugs for weight alone.
- Clinics serving low-income patients, who must plan alternatives.
The wider trend
Several public and employer plans reviewed or limited coverage in 2025 for the same reason. Price cuts later in the year changed the math for some of them.
